Before School Starts

☑️Health Forms - Health Forms and Permissions in Magnus are now overdue!  Students will not be permitted to come to campus until all health forms, including the Daily Health Attestation, are submitted. Reminder: A physical exam (PE) form dated after 8/1/2020 will meet enrollment requirements for the start of the 2021-2022 school year. If your child’s physical exam is set to expire during the upcoming school year, please remember to upload the new physical form to Magnus as soon as it is available in order to keep your child’s school health record up to date. Students who plan to participate in team sports must have a PE on file that is no older than 13 months from the start of the season.
☑️Health Attestation - Complete the Daily Health Attestation in the Magnus Health program stating your daily commitment to adhere to the safety policies and procedures that will help keep students, staff, and faculty safe.

☑️COVID Vaccination - If your child is 12 years old and therefore eligible for the COVID vaccine:  a) If your child has received the vaccine - please upload a copy of your child’s COVID vaccine card into their health record via the Magnus portal; b) If your child will receive the vaccine when they become eligible, please upload a copy of the card to Magnus once they are fully vaccinated; c) If your child is eligible but will not be receiving the vaccine, please notify your child’s school nurse:

☑️Masks - Purchase multiple masks for your child to ensure two (2) clean masks are sent to school each day (one to be worn and one as a back-up). Please send the back-up in a resealable plastic bag. We are following CDC guidelines for face coverings: Mask Guideline. Also, review with your child How to Wear a Fabric Mask Safely.

☑️Handwashing Safety - Please review handwashing and hygiene protocols with your child using this helpful Handwashing Guide. Alcohol-based hand sanitizer will continue to be available throughout our school buildings for use by students. Please notify the school nurse if your child cannot use school-provided hand sanitizer.

☑️Transportation Form - Complete the Transportation Form and confirmation (in Magnus) to let us know how your child will arrive at school and be picked up at the end of the day and who has permission to drop-off and pick-up your child. 
☑️Medications - Prepare your child’s school medications for the new school year. Check expiration dates and be certain that you have obtained new medication orders for the school year from your child’s healthcare provider and that you have completed a Parent/Guardian Authorization form for Prescription Medication Administration (in Magnus). Upload both of these completed forms to your child’s Magnus account. Please contact your child’s school nurse to arrange for delivery of the medication to school.

☑️Supplies - Carroll School will provide all school supplies; each student will have their own supplies to limit sharing of materials.
 

EVERY DAY

☑️Monitor Symptoms of Illness - Monitor your child daily for symptoms of COVID and notify the school when your child has symptoms or has been exposed to COVID, has tested positive for COVID, is awaiting results of a COVID test, or has traveled to high-risk areas. Use this helpful Daily COVID Health Checklist to know when to keep your child home and when to notify the school. Keeping students home when there is ANY possibility of sickness or exposure will go a long way in keeping school open this year. Together, we can prevent the transmission of COVID.

☑️Absent/Tardy Notification - Notify the school when your child will be absent, tardy or being dismissed early by completing the Absentee/Tardy/Early Dismissal Form.
☑️Travel - Review the updated Travel Policy on the COVID Health Guidelines & Protocols page.

☑️Sunscreen & Insect Repellent - Please apply sunscreen and insect repellent (follow package directions or instructions from your healthcare provider) to your child each morning before school. Our goal is to get outside as much as weather will allow. The Massachusetts DPH has noted that we should expect that EEE (Eastern Equine Encephalitis) and WNV (West Nile Virus) will continue to pose a threat in MA this season.

☑️Masks - Send (2) clean masks to school each day with your child in a resealable plastic bag. Mask Guideline

☑️Snack, Lunch, Water - Parents pack a snack daily and lunch (Monday-Thursday, and Friday if attending sports or the after school program) that does not require refrigeration or heating. There will be no food sharing of any kind, no use of a refrigerator, and no lunch service delivery. Students should bring a water bottle clearly labeled with their name. Water filling stations will be available; there will not be plastic cups or containers for use.
☑️Chromebooks & Chargers - For Grades 5-9, students should bring their Carroll-issued Chromebook and power cord to school each day. Please have Chromebooks fully charged at home each day before coming to school. For Grades 1-4, school-issued Chromebooks will remain in school.
☑️Carpools - If carpooling to school, we strongly suggest that unvaccinated drivers and co-passengers adhere to safety guidelines such as wearing masks, maximizing distance, opening windows for air flow (when possible), etc. Reminder: please do not drop off earlier than 7:45 am or pick-up later than 3:30 pm (unless your child is enrolled in sports or after school programs).
☑️Drop-Off & Pick-Up Procedures - Follow these Drop-off and Pick-up procedures for your child's campus to ensure safety of our community. Let us know if there is a change in transportation plans over the course of the year. Any transportation changes should be reported to us by emailing Mary Teixeira (Registrar).
☑️Contact Us - Please reach out to your child’s Division Head (LS - Sue Kingman, MS - Allison West, US - Kate Collins) if your child is struggling academically, socially, or emotionally. We are committed to partnering with you to give your child what they most need.
