How can I help you?

As the Carroll School Registrar, I am here to support you and your family throughout the year with documentation and forms.

I CAN HELP WITH MONTHLY ATTENDANCE REPORTS:

I can provide monthly attendance reports for any family that requests them. Email me (registrar@carrollschool.org) and I will put you on the list.

I CAN HELP WITH FORM REQUESTS:

I can help gather forms for doctors, school districts & private schools, etc. Please read through the process.

Key Points:

• If you are emailing forms, please email all forms to: registrar@carrollschool.org
• If you are dropping forms off or sending them to school with your student, please have them dropped off at the reception desk with the Campus Administrator.
• PLEASE do not send or give forms directly to teachers. And PLEASE do not share teachers’ email addresses with your district or doctor.

For your convenience,

• Here is the Release Form which must be signed before we can complete any forms.
• Here is a letter explaining Carroll’s involvement in the IEP process as well as a list of special services we provide for our students. I send these to districts when they ask for data for IEP meetings.

During the first month of school, when possible, forms will be completed by last-year’s teachers who know the student best.

During June, we ask you to try to avoid sending in forms. June is so hectic that sometimes teachers are unable to complete the forms before they leave for the summer.
