REQUIRED MATERIALS FOR GRADES 1-8

The Admission Committee will review applications once all materials have been received. Please feel free to include any additional testing or information you feel will give us a better understanding of your child's profile. Required materials are listed below:

Application Form

School Records

• Report cards and progress reports for the last two marking periods
• CORE evaluations and the Individual Education Plan, if available 
• Current achievement test scores

Diagnostic Evaluation Reports

• Neuropsychological, educational, and psychological evaluations which have been administered in the last year

WISC IV Testing

• Subtest scores and narratives, including overall cognitive ability, administered within the last two years

Speech and Language evaluation

• Full Speech and Language Evaluation required

Treatment summary from a therapist, counselor, or psychologist, if applicable
